Parent/Guardian Authorization

I hereby give permission for my child to attend Triangle YMCA Camp and to participate in all associated activities. I understand that the camp does not carry medical insurance and that I will be responsible for any medical charges my child may incur. The enclosed deposit is not refundable. I give permission for the camp administration to use photographs or videos of my child for promotional or other legitimate purposes.
